Off-screen, D’Onofrio has three children. He has a daughter, Leila, with actress Greta Scacchi, and two sons with model Carin van der Donk, whom he married in 1997 and divorced in 2023.

Now also an author and filmmaker, D’Onofrio continues to act, most recently in The Unforgivable and Dumb Money.
